Requesting a transcript

When requesting your transcripts, please include the following information in a signed letter written to the registrar office of the school(s) that you have attended:

  • That you need an official copy
  • Your full name at time of attendance, current name (if different)
  • Social Security Number
  • Current home address
  • Daytime telephone number where you can be reached in case of questions
  • Last date of attendance
  • Name and address of where you would like the transcript sent:

    * Be sure to call the school first to see if you have to send money for the transcript.
